Forklifts are critical to warehouse logistics performance, and any decline in their performance leads to increased downtime, higher maintenance costs, and operational disruptions. This study investigates the effects of Condition-Based Maintenance (CBM), Operational Environment Conditions (OEC), and Human Resource Competency (HRC) on Heavy Equipment Performance (HEP) of forklifts in warehouse operations. Using a quantitative, causal–explanatory design, data were collected via a Likert-scale questionnaire from 49 operators, technicians, and supervisors selected through purposive sampling. The data were analyzed with Structural Equation Modeling–Partial Least Squares (SEM-PLS) using SmartPLS 4.0, and all indicators met validity and reliability criteria. The model’s R-square value of 0.783 indicates that CBM, OEC, and HRC together explain 78.3% of the variance in forklift performance, measured by transportation productivity, downtime frequency, and maintenance cost efficiency. Hypothesis testing showed CBM has the strongest positive and significant effect on HEP (path coefficient 9.225), followed by HRC (5.999) and OEC (4.073). The study concludes that integrated improvements in CBM strategies, operator competencies, and warehouse environmental conditions can significantly and sustainably enhance forklift performance.
