Aspergillosis of the central nervous system is an uncommon infection, mainly occurring in immunocompromised patients. It may be presented in several forms: meningitis, mycotic aneurysms, infarcts and the tumoral form (aspergilloma). The authors report a case of a diabetic patient with cerebral aspergilloma. A forty-two-year-old woman presented with complicated diabetes, progressive headaches, and neurological deficits, which were assessed through imaging studies. A computed tomographic scan indicated a tumor-like lesion in the left temporal lobe, corroborated by magnetic resonance imaging that revealed significant infiltration. Surgery was performed to resect the lesion, which revealed a light yellowish appearance and hard consistency. This case emphasizes the need for awareness of cerebral aspergilloma in immunocompromised individuals, particularly those with uncontrolled diabetes, as it may manifest in a manner that mimics malignant tumors.
