Multimodal pain analgesia strategies are common in perioperative management of total knee arthroplasty (TKA). However, the comparative effectiveness of adductor canal blocks (ACB) versus femoral nerve block (FNB) on early postoperative recovery for revision knee surgery had not been thoroughly investigated. This study aimed to independently evaluate the impact of ACB on short-term postoperative outcomes, focusing on length of stay (LOS), postoperative narcotic utilization, and functional recovery during physical therapy in patients undergoing unilateral revision TKA. A retrospective cohort study was conducted involving 40 patients treated from January 2021 to July 2021, with 19 patients receiving ACB and 21 patients receiving FNB, all under spinal anesthesia combined with a standardized multimodal pain management protocol. The two groups were compared using independent sample t-tests, focusing on distance ambulated post-surgery and narcotic consumption. Results indicated that quadriceps strength was better preserved in the adductor group. Additionally, walking distance and stair-climbing performance were superior in the adductor group, while IV morphine usage within the first 48 hours was lower than in the femoral group. The findings suggest that adductor nerve block promotes better early recovery compared to femoral nerve block in revision TKA.
