Hydatid disease, caused by the cestode Echinococcus, particularly Echinococcus granulosus, is known to affect various organs, with common manifestations in the liver and lung. Although endemic in regions like Iran, the occurrence of primary hydatid cysts in the axillary region is rare, with only sixteen reported cases in the literature. This case report discusses a 33-year-old woman who presented with a mass in the axillary region persisting for four months. Axillary ultrasonography revealed a thick-walled cystic lesion, while mammographic and abdominal ultrasonography examinations showed no abnormalities. Surgical excision of the mass led to pathological examination, confirming a typical laminated membrane indicative of a hydatid cyst. Notably, postoperative IgG-ELISA serology results were negative. This case underscores the importance of considering hydatid cysts in endemic regions when diagnosing soft tissue masses in the axillary area, highlighting the need for awareness among medical professionals.
