In the Western world, the population has developed an overweight profile, leading to higher costs for the health system, especially for the morbidly obese. This study aims to correlate nutritional status according to BMI with the costs of laparoscopic cholecystectomy in a public hospital. Data were collected from medical records regarding nutritional risk assessment, nutritional state, and hospital costs for patients undergoing elective laparoscopic cholecystectomy. A total of 814 procedures were enrolled, with an average age of 39.15 years, where 78.3% were women. The cost averaged R$ 6,167.32 for a hospitalization duration of 4.06 days. About 68.4% of participants were classified with some degree of overweight, with a mean BMI of 28.07 kg/m², and 10% had a nutritional risk of 3 or more. A weak but non-significant correlation was found between hospitalization cost and length of stay, while a strong and significant correlation was observed for individuals with normal BMI. The results indicate that overweight patients incur higher costs compared to those with no complications, but lack a correlation with their nutritional status, contrasting with the strong correlation exhibited by patients with normal BMI.
