Ensuring quality performance in road widening projects is challenging due to numerous risks that arise during construction activities. This study aims to identify and prioritize significant quality-related risks in road widening projects using a Probability–Impact Matrix (PIM) approach. Focusing on the Taktakan–Gunung Sari–Anyer road widening project in Banten Province, Indonesia, risks were initially identified through semi-structured interviews with experienced practitioners and a structured literature review, then validated by senior experts. Nine key risks were assessed in terms of probability and impact on quality using expert judgment and plotted on a 5×5 PIM grid. The analysis reveals three dominant risks with very high significance: material supply delays, equipment breakdown, and material price escalation, all of which strongly influence quality outcomes through operational and procurement mechanisms. The study demonstrates the practical usefulness of PIM as a simple, transparent tool for prioritizing quality risks and offers guidance for improving quality risk management in road widening projects, particularly in data-constrained, developing-country settings.
