Expanding equitable access to healthcare for Medicaid beneficiaries and uninsured populations in the United States remains a major public health challenge, particularly in rural and underserved urban areas. Telehealth has emerged as a key tool to bridge gaps in care, yet its growth is constrained by regulatory, financial, and infrastructural barriers. This paper examines recent advances in public-private partnerships (PPPs) as strategic mechanisms to accelerate telehealth deployment for Medicaid and uninsured populations. It conducts a systematic review of federal and state initiatives, health system collaborations, and nonprofit–private sector alliances between 2015 and 2024, including case studies such as the FCC’s COVID-19 Telehealth Program, state Medicaid telehealth waivers, and partnerships between technology firms and community health centers. The analysis identifies five critical success factors: aligned policy and regulatory frameworks, shared infrastructure and data platforms, equitable funding models, community-based digital literacy and patient engagement initiatives, and interoperable technologies tailored to low-resource settings. Effective PPPs are found to increase telehealth uptake, improve chronic disease management, and reduce preventable hospitalizations in underserved communities. Persistent challenges include data privacy concerns, uneven broadband access, and fragmented policy implementation. The paper concludes that PPPs are an essential and scalable strategy for telehealth expansion, recommending stronger federal–state alignment, infrastructure investment, inclusive governance, and patient-centered approaches to promote sustainable, equitable care for vulnerable populations.
