Background: Endovascular procedures and direct surgical clipping are the main therapeutic modalities for managing of basilar artery aneurysms (BAAs). Furthermore, giant or wide-necked aneurysms and those that involve the PCA or perforators at its neck usually are not embolized. Objective: This study aims to present a modified transcavernous approach (TcA) for complex low-lying BAAs. Methodology: A 55-year-old man presented with sudden and intense headache, neurological examination revealed meningismus, and CT showed subarachnoid hemorrhage (Fisher grade III). Arteriograms revealed a BAA obscured by the PCA. A pterional craniotomy was performed, and proximal control was sought via drilling of the greater sphenoid wing and the PCA, facilitating exposure to the aneurysm's neck. Results: The modified TcA provided additional surgical room, allowed for successful proximal control and clipping of the aneurysm while preserving perforators. This modified approach demonstrated clinical usefulness in treating complex BAAs, emphasizing the need for tailored surgical techniques in challenging cases.
