Lower Recurrence Rate for Limberg vs. V-Y Flap for Pilonidal Sinus

Pilonidal sinus is a disease that lacks a standardized surgical treatment method. This study aimed to compare the outcomes of Limberg fasciocutaneous transposition and V-Y fasciocutaneous advancement flaps for treating patients with pilonidal sinus. A total of 111 patients (98 males, mean age 27.1 years) who underwent reconstruction after pilonidal sinus excision with Limberg flap (Group 1, n = 66) or V-Y flaps (Group 2, n = 45) between 1997 and 2004 were investigated retrospectively. No significant differences were found between Groups 1 and 2 regarding gender, infection history, operation duration, analgesic requirement, wound-related complications, or average time off work. The mean follow-up periods were 45.4 months for Group 1 and 48.8 months for Group 2. However, the recurrence rate was significantly lower in Group 1 (1.5%) compared to Group 2 (11.1%, P = 0.039). Based on these findings, we conclude that Limberg flap reconstruction is preferable over V-Y flap reconstruction in pilonidal sinus cases due to its reduced recurrence rate.
