The pathogenesis of lipomatous pseudohypertrophy of the pancreas remains unclear, but previous findings suggest a strong association with hepatic disorders, particularly in cases of chronic hepatitis and liver cirrhosis. This letter to the editor discusses a unique case involving a 32-year-old male diagnosed with primary sclerosing cholangitis (PSC) in 2001, who subsequently exhibited symptoms such as weight loss, watery diarrhea, pruritus, and scleral icterus. His family had a history of PSC, with his father undergoing liver transplantation. Initial liver biopsy indicated chronic hepatitis with bridging fibrosis and early cirrhosis, characteristic of PSC. Notably, this report highlights that, despite a significant relationship between lipomatous pseudohypertrophy and liver conditions, there have been no documented cases connecting this condition with PSC or instances necessitating liver transplantation prior to this case. This case adds to the limited literature surrounding the intersection of lipomatous pseudohypertrophy and PSC, providing new insights that may influence future clinical considerations and management strategies for similar patients.
