College students around the world are experiencing increasing mental health challenges, and AI-based mental health applications are being adopted to provide scalable support. This systematic literature review synthesizes qualitative studies published between 2020 and 2025 on college students’ needs, preferences, and concerns when using AI-enabled mental health tools such as chatbots, adaptive modules, and mood trackers. Five dominant themes emerged: personalization and adaptability; privacy and confidentiality; accessibility and usability; emotional support and human-like interaction; and integration with campus ecosystems. Students highlighted the importance of transparent data practices, co-design processes, culturally sensitive content, and clear pathways linking digital tools to campus counseling and crisis services. The review identifies gaps including a lack of longitudinal evidence, limited research in low- and middle-income contexts, and minimal attention to intersectional differences. It concludes that effective AI-based mental health tools for students must prioritize participatory design, privacy and explainability, and close integration with institutional mental health services, supported by future longitudinal, cross-cultural, and equity-focused research.
