This article provides a framework for the evaluation of shifting services to persons with severe mental disorders from state hospitals to community programs. A case study of the closing of a state psychiatric hospital in Pittsburgh, Pennsylvania, and the scaling down of other facilities in the area is used as an illustration. The proposed framework offers answers to long-term management questions and aids in policy development through the utilization of integrated systems and client-specific outcome data. The study focuses on management questions regarding the effects of reducing reliance on state hospital services, designing evaluations for major changes in urban service systems, and the associated policy implications. Long-term issues considered include the necessity of state hospitals for the care of persons with severe mental disorders, the efficacy of treating these individuals in non-institutional settings, and the economic, clinical, and service system ramifications of closing state hospitals.
