The development of the Electronic Health Record (EHR) application for the surgery center integrates two collaborating hospital service units: surgery and anesthesia. Recognizing that prior work typically focused on these units separately, this study aims to build a unified EHR application that supports combined surgical and anesthesia workflows within a Surgery Center. The system was developed through sequential stages of requirements analysis, design, construction, and implementation, involving stakeholders from general and psychiatric hospitals in Riau Province, Indonesia. Key features include registration of surgical and anesthesia patients and structured documentation for pre-operation, pre-anesthesia, intra-operation, intra-anesthesia, post-operation, and post-anesthesia phases. The application was prototyped iteratively, refined based on user feedback, and tested in a regional hospital to validate functionality and usability. Implementation activities included comprehensive testing, user training, data migration, and deployment preparations. The resulting integrated EHR application is intended to facilitate and improve the effectiveness of completing medical record data in surgical service centers.
